Request for the Administration of Medication
St Mary’s College acknowledges that arising from the duty of care that the College owes to its students, there will be occasions when the administration of medication may be necessary to support students during the course of normal College activities. St Mary’s College is committed to assisting families with optimising the health, safety and wellbeing of its students.
To ensure correct procedures are maintained, we request the following form be completed as soon as possible so that St Mary’s College staff may begin to administer medication to your child during the school day. The form is designed to help ensure the health, safety and wellbeing of your child and to assist College staff, as they do not have medical training beyond first aid.
The form should be completed with assistance from the prescribing medical practitioner or the pharmacist dispensing the medication and must be accompanied by an appropriate Action Plan for conditions such as Asthma and Anaphylaxis.
Once the form has been completed and the medication provided, the College staff can only then begin to assist your child with the administration of medication.
Parents are responsible for:
- Obtaining the relevant medication forms from the College and arranging for their completion and return where the use of the electronic form is not preferred and;
- Providing the medication in the original labelled container to the nominated staff member and;
- Ensuring the medication is not out of date and where prescribed, has an original pharmacy label detailing the student’s name, dosage and time to be taken.
