Authorisation for Administration of Student Medication
St Mary’s College is committed to ensuring the health, safety, and wellbeing of its students, with additional safeguards in relation to medication. Where student medication administration is required during school hours, the following conditions must be met.
- The medical condition related to the medication must be listed in Consent2Go with clear information around administration times, dosage etc.
- The Authorisation for Administration of Student Medication form must be completed by the family as well as the relevant nurse/chemist/ prescribing doctor
- Medication must be supplied by an adult to Student Services in the original chemist box or Webster Pak/chemist-labelled bottle (half tablets, titration etc.) each time, e.g., no loose sheets or zip-loc bags.
- Medication must be signed in and out by an adult as required (end of term or changed dosage etc.)
Where an Action Plan or Individual Health Plan is required (asthma, anaphylaxis, epilepsy, diabetes, CF, etc.) families must ensure that a current version is supplied.
